自我追蹤與健康管理:穿戴裝置中介健康的理論分析架構

智慧穿戴裝置因具有收集、監測身體數值的功能,已日漸成為人們追蹤和管理健康風險的工具。然而,隨著裝置的普及也出現了一些隱憂,包括醫病關係轉變成醫療專業—物—使用者的關係,仰賴客觀數據而導致的主觀身體意識降低,以及數據持有與近用不平等而導致的數據商品化現象。本文回顧現有文獻,區分成從權力視角切入的政治經濟學和傅科式權力理論,以及從行動與身體經驗角度切入的STS (Science, Technology, and Society)和後現象學,來探究技術中介下所帶來的多重社會效應。本文以偵測心房顫動的「愛心鏡」APP 為例,提出一個基於多重理論的整合分析框架,指出在探究智慧穿戴裝置的中介效應時,需同時關注醫病關係的權力運作、使用者主體經驗的改變,以及技術物在這些異質網絡中扮演的促動角色。本文提出的整合分析框架,有助於自我追蹤者、醫療專業者、學者和設備製造商來共同應對智慧穿戴裝置所導致的權力結構與身體意識變化,以深入省思此新興領域的發展方向。
英文摘要
Wearable devices, such as fitness-tracking watches, are widely used because they allow people to monitor their health and fitness using digital technology. However, with the growing use of these devices, effects have emerged, such as a shift in the doctor-patient dynamic to a doctor-object-patient relationship, a reduction in subjective bodily awareness due to reliance on objective data, and data commodification resulting in unequal data ownership and access. This article reviews the existing literature on the subject and distinguishes between two perspectives: first, the political economy and Foucauldian power theory, which approach the issue from a perspective of power; and second, STS and postphenomenology, which approach it from the standpoint of action and bodily experience. By exploring the multiple social effects brought about by technological mediation, the article proposes an analytical framework and applies it to examine the social effects related to Rhythm Cam, a smartphone app that allows patients to independently monitor their heart rhythm for abnormalities. Our proposed analytical framework highlights the need to simultaneously examine the power dynamics in the doctor-patient relationship, the changes in the subject experience of patients, and the role of technological objects in these heterogeneous networks when investigating the intermediary effects of smart wearable devices. By proposing this analytical framework, the article aims to call on self-trackers, physicians, scholars, and device manufacturers to address the changes in power structures and bodily awareness brought about by smart wearable devices and to deeply reflect on the future development of this nascent field.
