戰前日本海外醫療設施在菲律賓建構的過程:兼論臺灣總督府之角色

本文由戰前日僑向海外移民所帶來的醫療問題為發端,以東南亞 日籍移民數量最多的菲律賓為例,探討日本醫療設施在海外建構的發展 歷程,並兼論作為日本「南進基地」的臺灣,如何居中扮演協力者的角 色。首先、說明日本中央對於移民醫療需求的處理辦法及相關規程,以 及在菲日人醫療設施的建置概況,並究明菲律賓當局對日人醫療設施在 當地設置的反應,再者,擬由菲律賓的醫療衛生概況談起,說明美領菲 律賓的醫療體系建構情形及當地醫師開業規則,以釐清菲律賓當局對外 籍醫師及醫療設施之態度。最後將討論重點置於臺灣總督府的措施上, 探究臺灣總督府如何藉由提供補助經費及補充醫事人才的方式,協助建 構在菲律賓的日本醫療設施。透過本文的討論,除了釐清日本在菲律賓 建構醫療設施之過程及發展情形,並說明臺灣總督府在此過程中扮演的 角色外,也點出日本在菲律賓所設置的醫療設施,其意義已不僅止於提 供日僑醫療所需之用,實際上已成為日、美、臺、菲之官方角力及民間 交流之場域。
英文摘要
This paper focuses on the development of Japanese overseas healthcare infrastructure before the second World War.The main geographical focus for this study is the Philippines, which had the highest concentration of Japanese immigrants in the southeastern Asian region before the war.This paper also takes Taiwan into consideration,as it is the Japanese“Southern Advance”command base, and faciliated pre-war healthcare programmes. The first section focuses on procedures and regulations of immigration healthcare published by the Japanese government proper,as well as the infrastructural investments made by Japanese expatriots in the Philippines. How the Philippines authority responded to these Japanes infrastructural initiatives will also be addressed in this section. The second section gives an account on the healthcare system and medical practice regulations stipulated by the United States-led administrative authority in the Philippines,in order to reconstruct the point of view which the Philippines authority held regarding foreign healthcare personnel and infrastructural investments.The third section discusses how the office of the governor-general of Taiwan supported the Japanese healthcare programme in the Philippines,particularly on finances and medical personnel.This paper clarifies the process of development,as well as the historical significance,of Japanese healthcare investments in the Philippines, along with the degree of involvement on the part of the office of the governor-general of Taiwan.This paper also argues that the Japanese healthcare investments and infrastructures in the Philippines had exceeded its designated role of providing healthcare services to Japanese expatriots,They collectively form the space for bureaucratic and civil exchange and interaction between the four parties involved-Japan, U.S.,Taiwan,and the Philippines.
